These teams are part of NELFT NHS Foundation Trust and provide specialist, evidence-based care. Whether you're managing your mental health or experiencing a crisis, the teams are trained to help you work towards your recovery and independence.
About this service
Three specialist mental health teams across Barking & Dagenham serving adults aged 18+ with serious mental health problems. Each team includes community mental health nurses, occupational therapists, psychiatrists, psychologists, and support workers. Services include: - Medication management, depot and clozapine clinics - Physical health screening and health maintenance - Psycho-educational interventions - Access to psychological therapies - Support with employment, education and training - Links to NELFT recovery college - Carer's assessments and support
How they can help you
The teams assess your individual needs and work with you to develop a personalised support plan based on the Recovery Model, promoting mental wellbeing and independence through community interventions and evidence-based approaches.
What to expect
You'll be assessed for your individual needs and work with the team to create a support plan. The multi-disciplinary team may include nurses, therapists, psychiatrists and support workers who'll work with you based on Recovery Model principles.
