The Early Intervention and Prevention Team across Perth and Kinross offers assessment and access to community and day support that helps older people remain independent. The team works with older people and their families to understand what matters to them and find the right support early, before small needs become bigger. The aim is to help people stay well, connected and living the life they choose, for as long as possible.
